In Deltarune Chapter 3, you can only use the manhole if you get a secret rank called Z Rank.
This is different from the normal ranks like A, B, C, or S. To get Z Rank, you have to play badly on purpose.
In battles, take more than 10 turns, let enemies hit you a lot, and finish with a low TP bar.
How To Get Z Rank For Manhole In Deltarune Chapter 3
On the game board section in Chapter 3, try to end with the least amount of coins. A good way to lose coins is by dying in battles, because it lowers your total coins.

In minigames, don’t try to win. But you also can’t lose fully, or the game will make you try again.
So just get a very low score without failing. The goal is to do badly in a smart way. If you do all of this, you will get the Z Rank.
How To Use The Manhole
After you finish the board sections with a Z Rank, go back to the ranking area, where the A, B, and C Rank doors are.
Now, a new Z Rank door will appear. Go inside, and you’ll find a secret room. After that, the manhole will open, and you can go through it to reach the games room.

The Z Rank path is a hidden part of the game that you can explore if you play differently.
Even though the Z Rank path feels different, it still joins the same story path as the S Rank one later on.

There’s also a theory that you might be able to do both the Z Rank and weird route by paying an NPC to upgrade your rank to S after finishing the Z section.
The Z Rank and manhole path are a secret that rewards players who try something different.
